I forked this project from Solid/form-playground, it was working as a stand alone app on any webpage but then quit working all of a sudden. The Mashlib.js that was used had been deleted, luckily, I was able to download the entire contents locally and was able to find the missing code. https://mikeadams1.github.io/form-playground/playground.html
